What about the thing we call laziness or slackers. I guess I could be more PC and call it procrastination.
I think we're all familiar with this problem that is plaguing not just doctors implementing EMR. It's just easier to wait.

Interesting but the most important one, the one that nobody likes to talk about, is liability.
Clinicians, in general, are suspicious of "over documentation". Many clinical professionals have suffered painful legal experiences, where their own documentation has been used against them.
Perhaps this fits into the "war stories" category, but I believe that legal and liability issues are still very high profile, and a significant mitigating factor in this country's progress in health care technology.
